Benign Pigmented Lesion Treatment
Treatment of facial melasma is controversial as it is a condition controlled by hormones and may often reoccur. Furthermore, the response is unpredictable, presenting no response, lesion lightening, or even lesion darkening.

Contraindications

01 Hypersensitivity to light in the near infrared wavelength region
02 No sun exposure or artificial tan for 4 weeks prior to or after the procedure
03 Taking medication which increases sensitivity to sunlight
04 Light induced seizure disorders 
05 Active localized or systemic infections or an open wound in the treatment area
06 Pregnant and/or breastfeeding 
07 Significant systemic illness or an illness localized in area being treated 
08 Common acquired nevi that are predisposed to the development of malignant melanoma
09 Herpes simplex in the treatment area 
10 Are receiving or have received gold therapy
11 Unrealistic expectations of end results

Cautionary Criteria & Precautions

01 Unprotected sun exposure within 4 weeks of treatment. This includes tanning beds, tanning products, creams, lotions, and sprays 

02 History of immunosuppression/immune deficiency or auto-immune disorder 

03 Coagulation disorder, on anticoagulant medication including aspirin

04 Medications which alter the wound healing response 

05 A history of keloid formation 

06 A history of skin cancer or suspicious lesions in the treatment area

07 If the tattoo is the result of gunpowder or a combustible element 

08 Botulinums (Botox, Xeomin, Dysport) 2 weeks prior, or cosmetic dermal fillers 3 weeks prior or implants in the treatment area 

09 PCOS or risk of paradoxical hair growth

Patient Pre-treatment Guidelines

01Test spots are recommended prior to treatment to determine safe and effective treatment parameters for darker skin complexion (olive and darker) 

02Avoid irritant topical agents, acne treatments (use only as a spot treatment), retinoids, bleaching/brightening ointments, serums, acids (EXAMPLE: vitamin A, vitamin C, hyaluronic, lactic, salicylic, alpha-hydroxy, glycolic acids) 10-14 days before treatment.

03Laser Hair removal, waxing/sugaring, hair removal creams must be done 14 days prior to treatment and after

04 Determine if a cosmetic tattoo is present in treatment area. If present, treat at least 2 fingerbreadths away from tattoo.

05Area should be clean shaven 24 HOURS prior

06 Pretreatment photos are strongly recommended

01Slight swelling (edema) and erythema (redness) of the treated area may occur after treatment 

02Cool the skin as needed with cold gel packs, cool gel, or cool air

03Apply pure hydrocortisone 1% 2-3 times a day mixed with clear aloe vera gel for first 24-72 hours.

04 Healing/Recovery serums are recommended to apply frequently to create protective skin barrier and moisturization

05There may be a slight darkening (bronzing) of the skin noticeable 3-14 days posttreatment which resolves without intervention.

06 Moisturizer and make-up may be applied after each treatment if skin is intact, no redness or any irregular skin reaction.

07For patients who are prone to breakouts or have sebaceous skin, consider waiting 24 hours before applying any topical products.

08Avoid sun exposure between treatments. If sun exposure is unavoidable, apply SPF 30+ to protect exposed, treated areas

Professional Treatment

MELASMA Reported adverse events included: mild erythema, itching, desquamation, dryness, pruritus, scaling, hyperpigmentation, macular hypopigmentation, and instances of melasma recurrence.
